Documentation of an accident or incident was insufficient in that it did not include the [a parent/guardian signature on four forms and a date (including year) on two of those forms]. CCF Handbook, Section 6.4, C (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: The four children listed on the Supplemental Sheet had incomplete Accident/Incident Reports. The director stated that he will ensure the Accident/Incident Reports are completed with a full date and time, as well as a parent/guardian signature prior to filing them to prevent this from occurring in the future. Technical support was provided. 6.4 Accident/Incident Notification B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/childcare personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. 04. Supervision CCF Handbook, Section 2.4 (04-01)

Supervision of children in the [VPK class] group was inadequate in that [a child was left in the bathroom and a check off of attendance was not conducted,]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: During todays inspection a child in the VPK class group was left in the bathroom while the class went to the playground. At the time of inspection, the teacher went back to the classroom and brought the child outside with his VPK class. Technical assistance was given on checking off attendance before and after the playground. Technical assistance was given. CCF Handbook, Section 2.4 2.4.1 General Supervision Requirements A. Childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. A ratio of [1 ] child care personnel for [4 infants] children is required. A ratio of [1] child care personnel for [1 infant and 6 two year olds] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: At today's inspection the two-year-old room was observed with an insufficient ratio: During naptime there were 6 two-year-old children and 1 infant present. • The director called the infant teacher who was on lunch break to return to the center.
