Designated Representative stated personnel records are now maintained and stored electronically. Director was unable to access or gain access to electronic records for licensing to review. Director did have an account or password to access personnel records electronically at the time of inspection. Licensing Specialist assisted Designated Representative who was virtually trying to assist Director create account however Director was unable to access electronic records. See supplemental. The playground is currently unusable due to remaining concrete debris that needs to be cleared. Once the cleanup is completed, fall zones will need to be established. Documentation was not maintained for 12 months showing that routine inspections were conducted monthly of all supports, above and below the ground, connectors, and moving parts. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. The physical environment log has on been completed since 11/18/24.
