During file review Licensing Specialist observed 2 personnel files that did not have a current CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed. Corrected prior to uploading. Technical assistance was provided regarding personnel annually signing CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form. Licensing Specialist received email from provider on 04/23/2026 with personnel completed child abuse and neglect reporting forms, email deleted.
