Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Comments: Allegedly a child has bruises and marks that are unexplained and no documentation was given to the parent. The licensing counselor analyzed several hours of video footage. The child was seen descending the slide on the playground when several other children chose to come down the slide alongside her. It appears the child was kicked in the back by another child coming down the slide. After sliding down, the child approached the teacher to show her back. The teacher inspected the area and permitted the child to resume playing. The counselor received reports from the previous week regarding incident and accident reports, but there was none for that day on the playground. The teacher felt it was unnecessary to document the incident since she did not observe any marks on the child at that moment. The facility implemented a communication log to enhance communication with parents regarding interactions between children, teachers, and parents. Additionally, the facility is transitioning to a state standard ratio rather than a HeadStart ratio to ensure increased one-on-one visibility and interactions with children. 40. The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. The facility had gaps in the fence that posed hazards to the children. The director contacted the vice president of the facility to have this issue addressed by the next business day. Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Comments: A Teacher was observed repeatedly adjusting the child in his chair by pushing his seat in and standing behind him to prevent him from leaning back from the table. She physically pulled the child to stand up and walked him to the trash can, using her arms to guide his movements, and then returned him to the table. A short while later, when the child attempted to leave the eating area, the teacher grabbed his arm and led him back to the table. She used her body to assist him in cleaning his spot at the table, after which the child sat back down in his seat. The teacher repeatedly used her hands to turn him to face forward, pushed his chair in closer to the table, and stood behind his chair to restrict him from leaning back. Eventually, Teacher A moved away from his area. Per DCF Section 8.2, C Childcare personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited The facility held a child abuse training session to retrain staff and address burnout. A licensing counselor was present during the training to ensure that all current staff completed the session. CCF Handbook, Section 3.10.2, Comments: DCF Handbook, Section 3.10.2, When diapers, clothing or linens that are being used become soiled or wet, they are to be changed immediately, and properly disposed. The teachers in the room were unable to provide accurate diaper change times for the child that day. They used a whiteboard to record observations but failed to include timestamps for the child's diaper changes. In response to this, management has replaced the whiteboard with daily paper sheets for parents and teachers to sign off on diapering.
