30U — Personnel Files

Ensure staff files are complete. Nine staff need CPR with skills demonstration. One staff needs annual review of child abuse/neglect, orientation, OCCL's Health and Safety Training, employee declaration and drug/alcohol statement, adult abuse registry check, and release of employment history. One staff needs OCCL's Health and Safety Training, an application or resume, release of employment history, and references. One staff needs an annual review of child abuse/neglect, orientation, OCCL's Health and Safety Training, application or resume, release of employment history, and two more references. One staff needs review of child abuse/neglect, orientation, OCCL's Health and Safety training, job description, references, employee declaration and drug/alcohol statement, and release of employment history.

40U — Child Files

Ensure child files are complete. One child needs a health appraisal.

53U — Outdoor Area

Ensure exit gate on outdoor play area is unlocked while children are in care. This was corrected at the visit when the gate was unlocked and the center agreed to comply in the future.

77U — Infant and Toddler Care

Ensure styrofoam objects are not used with infants and toddlers. Replace styrofoam food service products with an acceptable alternative.

Ensure staff files are complete. Two staff needs a signed professional development plan. One staff needs an adult health appraisal. One staff needs a signed employee declaration. One staff needs an adult health appraisal with TB screening.

38U — Parents Right to Know

Ensure Parent's Right to Know log is reviewed with and signed by each newly enrolled family or any families touring the facility. Review and obtain necessary signatures.

40U — Child Files

Ensure child files are completed. One child needs emergency card information, including emergency transportation permission.

49U — Indoor Space

Ensure walls and surfaces are in good repair. Repair walls where the paint is peeling in the green and blue rooms. Repair baseboard in the blue room bathroom.

61U — Diapering and Soiled Clothing

Ensure staff utilize a non-porous surface for diaper changing. The diaper mat in the green room was ripped and a cloth cover was being used in the infant room. This was corrected at the visit when the mat in the green room was replaced and the cloth cover in the infant room was removed. Ensure staff and children's hands are washed after each diaper change. This Specialist observed a staff person change three children's diapers without washing his/her own hands or the child's hands. This was corrected at the visit when it was addressed with the staff person and they all washed hands.
