Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 03/03/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergecny Disaster for was complete at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The DES Contact form was completed at the time of the inspection. The Notice of Inspection Rights were provided to the Licensee at the time of the Inspection. The fingerprint clearance cards for 6 of 6 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: Ensure medication forms are complete. Ensure First aid kits contains all elements. Ensure admission and release signatures are complete. Ensure vinyl furnishings are maintained in good repair. Ensure paper towels are mounted. Ensure personal items are labeled correctly. Ensure playgrounds are maintained free of cob webs. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
