A Plan of Corrections is not being accepted at this time. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility representative at the time of the inspection. Ratios observed were as follows: Infant 1:4 Toddler 1 1:3 Toddler 2 1:4 Toddler 3 2:8 Preschool 1 (3s) 1:8 PreK (4s) 1 1:5 PreK 2 (4s) 1:11 There were 6 staff interviewed during this investigation. There were 2 staff files reviewed during this investigation. 2 of 2 Fingerprint Clearance card reviewed were valid via the DPS website search. The following documents were observed: staff attendance records, rosters, and an Employee warning. Others interviewed: The complainant. Upon completion of Complaint investigation 162248 it was determined from the Compliance Officers' observations and interviews that 3 of 3 allegations were substantiated.
