Compliance (Annual) The following deficiency was observed at the time of the Compliance Inspection conducted on 9/5/2025 and is subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Emergency Disaster Contact Form was emailed to the provider. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that the back of the Criminal History Affidavit is placed in the staff files. 2). Ensuring that paper towels are placed in a single-use dispenser. 3). Ensuring that the Statement of Service is updated with a suspension and expulsion policy. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
