Complaint The purpose of the inspection was to conduct a Complaint investigation 165927 on 4.15.2026. A full inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were as follows: Infants 2:8 1's 2:10 2's 2:15 3's 1:13 4/5s 1:12 There were 5 staff interviewed during this investigation. There were 3 children interviewed during this investigation. There was 1 staff file reviewed during this investigation. 1 of 1 Fingerprint Clearance card reviewed was valid via the DPS website search. The following documents were observed: Child's file, rosters, videos, and a staff file. Others interviewed: The complainant. Upon completion of Complaint investigation 156927 it was determined from the Compliance Officers' observations and interviews that the allegation was unable to be substantiated due to lack of sufficient evidence.
