Ratios observed were: Infants:2:6 1-year-old children:1:3 2-year-old children:2:15 3 and 4-year-old children:1:13 4 and 5-year-old children:1:14 There were 4 staff members interviewed during this investigation. Documentation observed was: Rosters and Incident reports Upon completion of the complaint #00152101 and #00152107 investigation, it was determined from observation, interview, and documentation that 1 out of 1 allegations were substantiated. The following deficiencies were observed and are subject to changes pending programmatic review.
