The following documents were reviewed: Rosters, Staff files Upon completion of Complaint #00157671 it was determined by observation and interviews that 1 of the 2 allegations was substantiated. 1 of the 2 allegations lacked sufficient evidence to be substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. Please complete the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies.
