Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 6/11/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Empower Self Assessment was emailed to the provider. The Emergency Disaster Contact Form was left with the provider on-site. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring all rosters are updated. 2). Ensuring diaper changing sinks are only used for diapering. 3). Ensuring end dates are updated on medication authorization forms. 4). Ensuring infants do not wear bibs in the cribs. 5). Ensuring the Emergency, Information, and Immunization cards are fully completed. 6).
