Compliance (Annual) The following deficiencies were observed at the time of the compliance inspection conducted on 02/10/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the notice of the Inspections Rights and a Small Business Bill of Rights to the person in charge at the time of the inspection. Please submit the Plan of Correction via the LMS portal within 10 days. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The following was discussed but was not limited to: #1 Ensure children's nap sheets are stored separately when not in use; #2 Making sure rugs are free from hazards; #3 Hand Sanitizer must be stored inaccessible to enrolled children; #4 The children's sensory table must be supervised at all times when used by children; #5 The playground gate must be maintained free from rust; #6 Medication forms must be completed with the end date; #7 The Statement of Services must include all 18 items; #8 Children's Record Cards must have the date of enrollment; #9 Staff must record times of arrival and times of departure daily; and #10 Ensure staff-to-child ratios are maintained.
