Compliance (Initial) The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 06/15/2026 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 1 of 1 staff member was verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Outdoor materials: Exposed sharp plastic located on sun-damaged trikes and dramatic play furniture will be inaccessible to enrolled children *Outdoor swings and climbing equipment will meet the fall zone requirements: 6 inches of loose sand, or a shock-absorbing unitary material *Fire extinguisher will be serviced if the indicator needle drops below the green zone or a minimum of annually (current expiration 08/22026) *Unused outlets will be covered with a safety cap *Each licensed classroom will have a lesson plan, daily schedule, and capacity posted *Paper towels will be available in the dispenser next to the diaper changing surface *Life-saving medication can be stored in the classroom and will be inaccessible to enrolled children *Fire Drills will be conducted monthly at different times of the day, and documentation maintained for 12 months *Staff and child attendance will be maintained for 12 months *The injury and illness log will be maintained for 12 months *Staff File documentation *Classroom rosters and Emergency Record Cards *Personal products will be labeled *Drinking water will be available indoors and outdoors *Main posting will include the name of the Director and their designee Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
