Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The following deficiencies were observed at the time of the Complaint Inspection conducted on 3/16/2026. A full inspection was not conducted at this time. Ratios: School-age children: 1:26 There were 3 staff members interviewed during this investigation. There was a phone conversation with an employee from the upper management. The Compliance Officer attempted to contact the complainant via telephone on 3/13/2026. The Compliance Officer was unable to speak to the complainant. Documentation observed included statements from staff members. Upon completion of the complaint investigation, it was determined from observation, interviews, and documentation that the two allegations were substantiated.
