Initial Monitoring The following deficiencies were observed during the Initial Monitoring inspection conducted on 4/02/2026, and are subject to changes pending programmatic review. Please submit the Plan of Corrections through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. A full inspection was not conducted at this time. Two of two Fingerprint Clearance Cards reviewed were valid through the DPS website. The following items were discussed, but not limited to: *Attendance and Release records are completed each day an enrolled child is present. *Hazards are inaccessible to enrolled children. *The facility is maintained in a clean condition.
