Please submit the written Plan of Corrections within 10 days of receipt of this Statement of Deficiencies. 4 of 4 Fingerprint clearance cards reviewed were verified to be valid through the DPS website search at the time of the inspection. The following items were discussed, but not limited to: *Maintain unused outlets covered with a safety plug. *The School-Age room must be maintained free from hazards. *Maintain current lesson plans. *Ensure toilet paper is mounted. *Ensure dress-up clothes are maintained in a clean condition. *Ensure sand under fall zones is maintained. *Ensure pacifiers are labeled with an identifier.
