Modification The following deficiencies were observed at the time of the Modification inspection conducted on 6/18/2025, and are subject to changes pending programmatic review. A full inspection was not conducted at this time. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following items were discussed, but not limited to: 1. Children’s sign-in and out records. 2. Children's Emergency Information and Immunization records. 3. Staff attendance records. 4. Accessible drinking water. 5. Monthly fire drills. 6. Classroom rosters. 7. Notification of pesticides. 8. Child abuse and neglect- mandated reporter. 9. Written approval for personal products supplied by the facility. 10. Children with special needs and in the individualized plan. 11. Discipline and guidance procedures. 12. Written accident, evacuation, and emergency plans. 13. Notifying parents and documenting injuries. 14. Illness and infestation log. 15. Medication procedures. 16. Transportation and Field Trip procedures. 17.
