Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/19/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit the specifications for the climbing structure on the playground as soon as possible. ***The monkey bars are not approved for use by children under the age of 5. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Children's Records must be complete with all required information. 2. Medications and labels on over-the-counter medications. 3. The equipment on the playground must be age-appropriate. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
