Compliance (Annual) The following deficiencies were found at the time of the Compliance Inspection conducted on 5/19/2026, and are subject to changes pending programmatic review. The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: *Ensure Emergency, Information, Immunization Record cards are complete ; *Ensure outlets are covered; *Ensure menu lists specific food being served to enrolled children; *Ensure the lesson plan in Room B6 is current; and *Ensure Criminal History Affidavits are complete with the back side of the form. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection.
