Complaint The purpose of this inspection was to conduct Complaint Investigation #00169656 on 5/14/2026. A full inspection was not conducted. Please submit the Plan of Corrections through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. The following ratios were observed: *Infants: 2:9 *Ones: 1:6 *Twos/Threes/Fours: 2:12 *Threes/Fours/Fives: 2:24 The Complainant was interviewed as part of this investigation. Three staff members were interviewed as part of this investigation. Documents review: *Rosters *Incident reports *Staff statements Two of two Fingerprint Clearance Cards reviewed were valid through the DPS website. Upon completion of Complaint #00169656, it was determined from staff interview, facility documentation, and the Compliance Officer’s observation, that one of two allegations were substantiated. One of two allegations lacked sufficient evidence to be substantiated.
