   Compliance (Annual) The following deficiencies were observed at the Compliance Inspection conducted on 1/6/2026, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection. 6 of 6 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Ensure the gap in the small basketball hoop court is repaired. 2. Ensure medical authorization forms are complete and accurate. 3. Ensure correct capacity is posted in each classroom. 4. Ensure all diaper changing surfaces are kept clear of items not related to diaper changing. 5. Ensure soiled linen containers are equipped with a liner. 6. Ensure staff files have all required items and staff qualifications are documented. 7.
