Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 1/27/2026 and are subject to changes pending programmatic review. A full inspection was conducted. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit a Plan of Corrections via the LMS portal within 10 days of receipt of the Notice of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Survey was discussed at the time of the inspection. The fingerprint clearance cards for 2 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. One staff member did not require a card, as they were not 18 years of age. During the exit interview, the following items were discussed, but not limited to: *Updated Annual Training hours *Renewing fingerprint clearance card, timeframes *Updated 10-day new hire training *Ensure parents sign the Emergency, Information, and Immunization Record *Nap time procedures, cot placement *Outdoor equipment maintenance Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
