Complaint The following deficiencies were observed at the time of case # 00090949 investigation on 10/3/2024 and are subject to change pending programmatic review, A full inspection was not conducted at this time. Compliance Officer: W. Manzelmann Ratios observed were: 1’s and 2’s: 2:12 Preschool: 2: 17 The complainant was contacted on 10/2/2024. There were 3 staff interviewed during the investigation. There were 2 staff files reviewed during the inspection. Documentation reviewed: written incident notes. Upon completion of the complaint investigation, it was determined from staff statement and documentation that one of two allegations was substantiated.
