   Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 5/18/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The DES Contact form was completed at the time of the inspection. The Notice of Inspection Rights were provided to the Licensee at the time of the Inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: Ensure the menu has components listed out. Ensure soiled diaper and soiled clothing containers have tight-fitting lids. Ensure to keep the Lego table in a clean condition. Ensure that toys and furnishings are in good repair. Ensure staff files contain 24 hours of training and new TB documents. Ensure Statement of Services reflects updated rule requirements.
