Four fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Complete and submit a Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. * Submit pictures of the items to be corrected in R9-5-603.C via email to the Compliance Officer. Items discussed, but not limited to: * Update personnel files with a new start date, Criminal History Affidavit, 10 day training form, fingerprint clearance card verification (DPS). * New DCS online procedure for background checks. * Menu - specify items being served and ensure menu is dated. * Sippy cups in toddler room. * Extra blankets on hand for naptime. Licensure is pending upon receipt and approval of pictures, complete drawings for the indoor and outdoor requirements. and the Plan of Corrections. The Department will notify you regarding the effective date of licensure.
