Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 08/15/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the facility at the time of the inspection. Please submit the Plan of Corrections in the LMS Portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form and the Empower Self-Evaluation link was emailed to the Provider. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Continue to monitor the playground to ensure hazards are inaccessible *Document a monthly fire and emergency drill *24 hours of annual training will be required *Stock medication will be removed from first aid kits *Emergency Information and Emergency Cards will be complete *Classroom temperatures will be maintained between 68 and 82 degrees. *Fans will be mounted Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
