Compliance (Annual) The following deficiencies were observed at the time of the annual compliance inspection conducted on 01-29-2026, and are subject to change pending programmatic review. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. Emergency card documentation. 2. Medication Consent Form documentation. 3. Food waste storage. 4. Playground maintenance. 5. Staff annual training requirements. 6. Staff file documentation 7. Classroom posting requirements. 8. Immunization exemption form requirements. 9. Suspension and expulsion policy--Statement of Services.
