746.1301(a)(5)(A) - Training Requirements for Employees, Caregivers, and Directors - 24 Annual training

Four caregivers were missing abuse and neglect training, one was missing SIDs and shaken baby training and medciation training, and one caregiver was missing emergency preparedness training, communicable diseases training, food allergy training, building and physical premises training, and handling hazardous materials training.

746.401(11) - Required Postings-Licensing notices with instructions to post

The operation did not post their permit renewal letter. This was corrected during the inspection when the letter was printed out and posted.

746.3803(a)(1) - Medication Authorization - Signed and Dated

Two children did not have medication authorization signed by the parents for their medication.

746.901 - Required Personnel Records

Out of 10 staff records reviewed, two were missing diplomas, two were missing pre-employment affidavits, one was missing a notarized employement affidavit, one was missing a statement that they completed orientation, and eight were missing copies of their photo identification.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

One of the caregivers did not have a background check submitted. This was corrected during the inspection when the caregiver left the operation.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

Monthly fire drills had been conducted, but children were unable to exit within three minutes. Fire extinguishers were also not checked monthly. The director checked the fire extinguishers during the inspection.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

One child had an allergy emergency plan that was signed by the physician but not the parent. This was corrected during the inspection when the parent signed the plan during pick up.

746.4003(a) - First Aid Kit - Incomplete Kit

The first aid kit was missing tweezers and a first aid guide.

746.3301(b) - Nutrition and Food Service - Follow Meal Patterns

The menu showed that children only received crackers for snack.

746.603(a) - Children's Records Maintained

Out of ten children's files reviewed, one was missing a physician's health statement, one needed updated immunizations, and nine were missing a date of admission.

One caregiver did not have first aid and CPR training and three caregivers' first aid and CPR training had expired.
