A caregiver stepped out of their room to open the door. This was corrected during the inspection when the caregiver returned to their room.

746.307(b)(2) - AP Parental Communication - Sign or Symptoms Requiring Hospitalization

During the DFPS investigation, there was sufficient evidence found to determine that a child's parent was not notified when a mosquito bite on the child's face that had occurred at home became swollen and required medical attention.

746.603(a) - Children's Records Maintained

Out of the ten children's files reviewed, eight were missing a date of admission, one was missing emergency contact information, one needed an updated immunization record, two were missing permission to ride the bus from school, and one was missing a statement that immunizations and hearing/vision screening results were on file at the child's school.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Four infants who were still on formula or breastmilk had feeding instructions that were not updated monthly.

746.201(1) - Policies Comply with Rules

The parent handbook was missing policies for doing morning health checks, applying sunscreen, and information on staff vaccinations.

746.1309(e) - Documented Annual Training- Required Content

Out of five staff files reviewed, three were missing annual training in abuse and neglect and the required content topics.
