The last fire inspection was observed to have been completed in February 2025. A fire inspection was scheduled during the inspection.

There was no documentation of the last fire drill available for review. The provider will conduct a fire drill during the fire inspection 5/4/26.

There was a garbage can in one classroom observed uncovered. There was food garbage observed inside and a lid was placed on the garbage can during the inspection.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

The was sufficient evidence found during the investigation to show that prohibited punishment was used when staff members yelled at children.

746.3407 - Maintenance of Building, Grounds and Equipment

There was sufficient evidence found during the investigation to show that the operation was not maintaining a clean environment.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

There was sufficient evidence found during the investigation to show that prohibited punishment was used when staff pulled or grabbed children aggressively.

746.2209 - Posting of Activity Plan

There were no activity plans posted in the operation.

746.1107(2) - Additional Minimum Qualifications for Caregivers - HS Diploma, Equivalent, Certification of Completion

4 out of the 8 staff files reviewed did not have a high school diploma or education verification in their file.

746.1301(a)(7)(A) - Training Requirements for Employees, Caregivers, and Directors - Transportation training

One of the four caregivers who were transporting had current transportation training.

There was no water at the operation all morning until around 12:00, noon.

Operation did not inform Child Care Regulation that the water had been turned off and that they were still operating with children.

Three of the four collateral parents that were contacted stated that when they dropped off their children that morning, they were not informed that the water was off.

A caregiver admitted to threatening to put a child in the closet if the child did not lay down and close their eyes during nap time.
