There was one caregiver present in a classroom with 5 infants ages 4 months, 9 months, 11 months, 15 months and 16 months old. An additional caregiver was added to the classroom during the inspection.

There was one caregiver present in a classroom with 7 infants ages 4 months to 17 months old. An additional caregiver was added to the classroom to be in compliance with the required child/caregiver ratio.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

One 4 month old infant was observed asleep in a pack n play with a blanket and an activity play mat. The items were removed from the crib during the inspection.

Two of three infant feeding instruction forms had not been updated within the required time frame of 30 days.

The outdoor space was observed to have an excess amount of dead leaves piled up in the play area posing a fall hazard.

Dated activity plans were not posted in the classrooms.

There was one peice of rotating equipment observed on the outdoor area without the required loose fille installed.
