A caregiver was aware of how many children were in the group at the playground but did not accurately count them all when leaving the playground back to class, resulting in one child being left behind.

Based on the information obtained, it was determined that a child was left unsupervised at the playground before another classroom caregiver found the child and had the child returned to their class.

1 child's immunization exemption form was incomplete, as it was not notarized. The same child still did not have a health statement on file.

This standard was found deficient as part of a DFPS investigation. An incident requiring notification was not reported in June of 2025.

This standard was found deficient as part of a DFPS investigation. The operation's supervision procedures for certain age groups was determined as not sufficient enough to provide visual or auditory awareness and proximity to children during restroom use. Children were allowed to walk from their class to a restroom far enough away from the class where the caregiver was not able to maintain adequate visual or auditory supervision.

A child was unintentionally left unsupervised in the gym area for approximately 2 minutes when the rest of the class had transitioned to the outdoor playground.

2025 fire drills were not documented, as required. Second half of 2024 fire drills were also not documented. Director stated the fire drills were conducted but not documented.
