746.3701 - Safety - Areas Free From Hazards

After being aware of a child who was able to open the playground gate with ease on more than one occasion, immediate action was not taken to better secure the gate. Video of an incident was observed where the child opened the gate and ran before being retrieved by the caregiver on 3/25/26. At the time of the investigation inspection on 4/2/26, it was observed that a correction to better secure the gate had not yet occurred, which showed continued risk.

746.603(a)(4) - Children's Records - Immunizations

2 children's immunization records did not have documentation of all required immunizations.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

Fire drills were not conducted for September and December of 2025.

746.1311(d) - Director Annual Training - Caring for children under 24 months

The director did not have current training in SIDS, Shaken Baby Syndrome and Early Brain Development.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

While redirecting a child's behavior, a caregiver grabbed onto a child for approximately 44 seconds.

746.1203(4) - Provide care consistent with child's habits, interests, strengths, special needs, including any special supervision needs

Care being provided was not consistent enough to address a routine occurrence of aggressive biting by a child in care. As a result, several children were being bitten on a routine basis, sometimes causing bite marks that lasted at or longer than 24 hours.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

A caregiver did not use good judgment or competency in how they communicated with a parent after their child received an accidental injury that was above minor. The parent was provided with insufficient detail of the injury and did not indicate how serious the injury was. As a result, the parent was not able to accurately access their child's injury and that it needed more immediate attention than waiting until pick up time. The child had several abrasions across the face, a bruise and later began to display signs of a possible concussion prior to being picked up by the parent. The caregiver also did not ensure that the director, who was still present at the time, was properly notified. The director was not made aware of the incident until after the child was picked up by the parent.

746.1003(2) - Director Responsibilities - Employees Comply with Standards

A caregiver did not comply with minimum standard rules regarding proper communication with the director and parents for more serious child injuries.

746.4803 - Measurement of Use Zone for Stationary Equipment

3 newly installed pieces of active play equipment had overlapping use zones.
