During the inspection an infant who could not roll over independently was observed in the crib face down.

This was determined to be deficient due to a toddler being left alone and unsupervised in a classroom.

The operation failed to notify the parent immediately of a non-routine situation that may have placed a child at risk for injury or harm.

The operation failed to report the incident to CCR within the required timeframe.

During the inspection there was a tall bookshelf in a classroom that wobbled when touched and was not anchored. This was corrected at inspection when the operation removed the shelf from the classroom during the inspection.

746.2911 - Adequate Lighting During Naptime

During the walk through a classroom was observed with inadequate lighting to be able to properly supervise the children while sleeping. This was corrected during the inspection when staff turned on classroom lighting.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

During the inspection three out of ten staff evaluated did not have current CPR training.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

During the inspection two out of ten staff evaluated did not have current First Aid.

During the inspection three out of ten staff did not have documentation they received an orientation.

During the inspection the operation did not have a medication listed on a Food Emergency Plan.

Four out of ten children's records reviewed did not have current immunization records on file.

During an inspection operation was observed not following a safety plan implemented by CCR and DFPS.

Sole caregiver in two year old classroom reported having 11 children. Caregiver was present with 13 children.

Sole caregiver was observed with eight infants with a specified age of 17 months. Corrected when a second caregiver was added to the classroom.

746.2911 - Adequate Lighting During Naptime

Three out of thirteen rooms were observed without adequate lighting during nap time at time of inspection.

Two infants were observed at time of inspection for over 15 minutes in restrictive devices.

During an investigation inspection it was determined cribs in the infant classroom with children under 9 months old are stacking mattresses for use of sleep.

During an investigation it was determined caregivers were on personal cell phones while supervising children.

During a DFPS investigation it was determined a child was left unsupervised outside for 10 minutes.

During an investigation it was determined director was not ensuring overall operation compliance.

During an investigation it was determined a tricycle without protective handlebar covers was accessible to children. Equipment was observed in active play area with children on video.

During a DFPS investigation it was determined the lighting in a nap room was not adequate at the time a child was left unsupervised.

This standard was evaluated as part of an investigation and found to be deficient when two caregivers failed to conduct name to face checks during a transition resulting in a child being left unattended outside for ten minutes.
