I conducted the walkthrough for indoor and outdoor. I observed two playgrounds for toddlers and preschoolers. There are some new wood chips under the equipment, but only 2 inches of loose fill (see picture)

I visited the operation for monitoring on 4/20/2026 and today. The Interim Director is not present both times. According to Admin, the interim director is only at the facility on Thursday and Friday. She is still in the process of moving from Pennsylvania to Texas.

During the inspection, I observed that a child's epinephrine had expired on 3/2026. An assistant called the mother to request the medication.

During the walk-through, I observed that neither of the outdoor playgrounds has enough loose-fill surfacing (Less 1 inch)

During the HASA review of 10 children s immunization records, 4 children did not have current immunization records.

The caregiver did not support the child's physical or emotional needs when the child got hurt.

The caregivers did not use positive methods of discipline and guidance to improve the behavioral concerns.

746.1203(5) - Responsibilities of Caregivers - Interact positively with children

The caregiver did not interact positively with the child.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

A caregiver was grabbing a child when the child was upset.

746.501(a)(23) - Written Operational Policies - Procedures parents access minimum standards online, TX A/N hotline, and HHSC website

Instructions on how a parent may access the Minimum standards on line, Texas Abuse and Neglect Hot line; and HHSC website were not available for review at time of inspection in the operation policies.

Operation policies stating the promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter was not available to review at time of inspection.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

During an investigation it was determined two infant bottles were not labeled. This resulted in an infant given the incorrect bottle and having an allergic reaction.

It was determined during an investigation an infant was served a food item identified as a diagnosed food allergy resulting in emergency medical treatment.

Individual without an active background check was observed alone with children.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Three children were observed alone in a room without a qualified caregiver at time of inspection. This was corrected when children were brought to a classroom with a qualified caregiver.

746.603(a)(1) - Children's Records - Enrollment Agreement

Two children observed at operation did not have enrollment paperwork on file. Corrected when enrollment forms were filled out at time of inspection.
