This standard was evaluated as part of the investigation and was found to be deficient. In two restrooms where diapers were changed, there was not a changing table or a smooth, non-absorbent pad to change diapers.

746.3501(1) - Diaper Changing Steps- Prompt

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with the parent and staff, it was determined that a child did not have their diaper promptly changed.

746.501(a)(20) - Written Operational Policies - Parent Rights

The parent handbook was missing information about parent rights.

746.311(a) - AP Parental Notification of Deficiency as described in 746.309 within 5 Days

This finding was notified after the completion of a DFPS investigation. The operation was notified of a deficiency in Standard 746.1201(4) - Abuse and Neglect on 01/12/26 and failed to notify the parents of the deficiency by the allotted date, 01/17/26. The operation didn't notify the parents of the deficiency until 02/06/26.

746.601(b) - Provide Access to Children's Records - Parents

This standard was tasked over for this investigation and found to be deficient. It was determined that a custodian of the child in care who is granted rights to the child's educational record by court order was denied access to the child's file. This information was requested multiple times.

746.1201(4) - AP Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited

Based on information provided from a DFPS investigation it was determined that a child in care was abused by a caregiver and the caregiver mentioned in the allegations has been found Reason to Believe.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Based on information provided from a DFPS investigation it was determined that a caregiver grabbed a child by the arm and lifted them from the ground. The action by the caregiver caused substantial bruising to the child's arm.

746.2803(4)(C) - Positive Methods of Discipline and Guidance - Redirecting Behavior

Based on information provided from a DFPS investigation it was determined that a caregiver did not utilize positive statements when redirecting a child in care to move to another area in the room.

746.3301(f)(1) - Nutrition and Food Service - Drinking water available at every snack, mealtime, and during and after active play

This standard was tasked over for this investigation and found to be deficient. It was determined during this investigation that the child mentioned in the report was not given and/or offered water or a change of milk to drink through the school day. The child was allowed to only drink the same milk he arrived to care with throughout the day. the center director, has in place that the children's personal cups can only be filled with water at drop off time. Milk will be given throughout the day in disposal cups.
