746.3701 - Safety - Areas Free From Hazards

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined that the cover on a hose connection box was broken on the playground. The box was not blocked off from children when a child received a minor injury from the box.

746.4751(a) - Maintenance Procedures for Active Play - Inspect Daily

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined that the operation was not conducting daily checks of their active play areas.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined that caregivers and administrative staff were aware of a broken cover for a hose connection box on the playground; however, the caregivers did not have an increased level of supervision for the box and as a result a child received a minor injury from the hose connection box.

746.603(a)(4) - Children's Records - Immunizations

At the time of inspection one child did not have their most current immunization record, according to their current age, on file for review.

746.2426(a)(2) - Infant May Not Sleep in a Restrictive Device - Remove the infant from the device and place the infant in a crib as soon as possible

During the observation inspection at the facility two infants were observed asleep in restrictive devices and one was asleep on a boppy in a sleep sacks. The infants were not removed immediately from the devices and placed in their cribs but were said to had fallen asleep after they were fed. The babies were eventually removed from the devices or awaken after approximately 3 minutes after the caregivers were instructed to do so.

746.1315(b)(1) - CPR Training and Recertification Adhere to Guidelines-skill based

One caregiver on file only had online CPR/First aid training and certification on file for review. An in-person skills renewal had not been completed.
