One slide was observed to have a worn area towards the bottom creating sharp edges. The operation will not use the slide until repaired.

Two caregivers did not know the number of children currently in their classroom after the children had transitioned from a skills class.

One caregiver did not remove gloves and wash their hands between diaper changes.

Several items in the first aid kits located in the transportation vehicles were observed to be expired or incomplete.

There were no fire drills documented since July 2024. A fire drill was completed during the inspection.
