Based on information gathered during the course of the investigation, it was determined that a grounds check was not conducted on the day of the injury.

The caregiver supervising the child at the time of the injury was alone and had been employed for over 90 days, and did not have first aid.

During the inspection two children were observed coming from the gym to the cafeteria with no supervision. The caregiver in the cafeteria was no aware of the children's present and the caregiver responsible for the children was observed still inside the gym.

The operation does not have an accommodation plan for a child in care who is enrolled with an identified special need.

The caregiver supervising the child at the time of injury does not have a first aid card, and CPR is only online.

The operation's site director CPR/First Aid is expired. One zone leader that has been employed for over 90 days have online only CPR.

The site director has 6.5 out of 15 hours required for the 8/2023-8/2024 training year. The site director does not have any of the require topics in health and safety.

Based on information gathered during the investigation and video footage reviewed, it is determined that a caregiver and site director did not use good judgment by allowing a child to expose himself with only a shirt on multiple times following a toileting incident for about 20 minutes on and off.

744.2551(1) - Toileting Assistance- Prompt

Based on information gathered during the investigation, a caregiver did not intervene to assist with toileting while the child stood with fecal matter on his body and was wet for 20 minutes.

Based on information obtained during the investigation, the operation caregivers did not intervene to ensure that a child was appropriately cleaned.

During the inspection, the substitute site director had to perform site director duties, leaving the high school employee in charge of the group.

The operation's management team continues to allow high school caregivers to supervise charge alone. During the inspection a high school student was observed to be supervising a child alone.

There were two caregivers present during the inspection and either knew how many children they were responsible for.

The operation has not provided the parents of the previously injured child with an incident report.

Children were observed multiple times during the inspection being walking away from the group and out of caregivers sight. Children were observed transitioning alone. Children were observed in the bathroom fighting while in care.

The high school student was observed in charge of the operation for over 40 minutes.
