745.635(2) - AP Background check determination - If ineligible then you must immediately remove subject from operation and confirm to CBCU receipt of notice letter

It was determined a person with an ineligible background check was present at the operation and had access to children. The operation has since re-submitted the background check and the person is now showing as eligible effective today, 1/06/2026.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

One classroom had 12 children in care and the maximum age group for the classroom was 11.

746.301(2) - Notification of Change in Space

The operation did not provide notification to Child Care Regulation of a new classroom added to the capacity.

746.4907(a)(2) - Installation and Maintenance of Loose-fill Surfacing - Nine Inches if Equipment Greater Than 5 Ft.

The loose fill on the fall zones was observed to be less than the required depth of 9 inches.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver was present and observed caring for a group of children and did not have an active background check at the operation. The caregiver was sent home by the operation during the inspection.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

An 8 month old infant was observed grouped with children with more than an 18 month age gap. During the inspection, the operation placed the infant in the infant classroom.
