A caregiver was observed on her cellphone and appeared to be texting when I arrived in the classroom.

A caregiver stated she had 12 children in care and it was determined she had 13 children in care.

Based on the evidence it was determined a caregiver did not utilize good judgement when they placed a child on a seat that was not age appropriate.

The operation did not complete a fire drill in January. The last documented fire drill was completed in December 2025. A fire drill was completed during the inspection.

746.5307(a) - Inspect Fire Extinguishers Monthly

The fire extinguishers were not inspected for the month of January 2026. The fire extinguisher were inspected during the inspection.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

There were 2 infant feeding instructions that were observed to not have been signed and updated.

It was determined a child was left on the playground without the caregiver knowing he was not with the group and was with an assessor, who was not a caregiver. It was determined the child was left without a qualified caregiver for approximately 2-3 minutes.

A caregiver stated she had 10 children in care and it was determined she had 11 children in care.

746.4501(b) - Chairs - Safety Straps Used if Required by Manufacturer

A child was observed in a high chair and was observed to not be fastened. The child was removed from the high chair during the inspection.

746.5307(b) - Fire Extinguishers Serviced

The fire extinguishers have not been serviced recently, the fire extinguishers were last serviced in 2023.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

There were 2 infants observed asleep with a bib on in their crib.

746.2905(2) - Children Not Required to Sleep - May Not Confine in a Restrictive Device

A child was observed sitting in a high chair during naptime in order to avoid him walking around the room, disturbing children. The caregiver removed the child from the high chair during the inspection.

1 out of 6 children's files did not have a health statement on file for review.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

There were 2 classrooms that were not in ratio during the inspection. The first classroom had 18 children, with 1 caregiver and the specified age group was 12-17 months. The second classroom had 7 children with 1 caregiver and the specified age group was 12-17 months. Another caregiver arrived for classroom 1 and the second classroom, 3 infants went into another infant room, which reduced the risk to children.

The operation did not have a current fire inspection report. The last fire inspection report was completed on 1/23/24.

3 out of 6 files reviewed did not have an emergency authorization on file for review.

A caregiver's file was reviewed and the caregiver did not have any hours towards health and safety practices.
