The caregiver stated she had 16 children in care and it was determined she had 17 children in care.

746.3407 - Maintenance of Building, Grounds and Equipment

There were several sleeping mats that were observed torn.

746.1613(a)(4) - Naptime Child/Caregiver Ratio - Additional Employee Required

During the inspection it was observed there was only 1 caregiver present during naptime. There was not another caregiver in the building.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A child in care was observed left alone in the back classroom while the caregiver walked to the front of the building.

746.3703(c) - Not Under the Influence of Alcohol or Controlled Substance

It was determined a caregiver drank an alcoholic beverage during their lunch break and came back to the operation and cared for a group of children.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An infant was observed sleeping in a restrictive device (bouncer). The caregiver removed the infant from the bouncer and placed the infant in their crib.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

An infant was observed in a bouncer without the safety straps fastened.

746.5307(b) - Fire Extinguishers Serviced

The fire extinguisher was observed to have last been serviced in April of 2025.

There were 2 groups combined outdoors, the caregivers stated there were19 children and it was determined there were 18 children in care.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

There were 3 infants feeding instructions that were observed not to be up to date. One had not been signed since March 2026 and the other 2 have not been signed since April 2026.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a current fire inspection. The last fire inspecton was completed in March 2024.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

A parent documented on their child's file that there child had an allergy to peanut butter and there was no documentation of a food allergy plan for review, in the child's file.

A caregiver informed she had 4 children in care and it was determined she had 3 children in care. The children were all grouped together during outdoor play.

4 out of 7 caregivers reviewed, did not have documentation of orientation completed.

746.501(a)(18) - Written Operational Policies - Promotion of Physical Activity

The operational policy was missing the following sections:Promotion of indoor/outdoor play, Emergency Preparedness Plan, Nursing Mothers, Preventing Responding to Abuse/Neglect, Employee Vaccinations, Procedures for Inclusive Services to Children with Special Needs, Gang Free Zone and Parent Rights.

3 out of 7 personnel files did not have documentation of pre-service completed prior to the caregiver being in a classroom.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

The operation did not have an infant feeding instructions on file for 1 infant. During the inspection, the parent came to sign and complete the form.

746.2411(2)(G) - Safety Requirements for Play Yards - Size of Mesh Openings

A play yard was observed to have a hole in the play yard and was bigger than 1/4 inch.

746.605(11) - Required Admission Information - Emergency Medical Authorization

A child's admission form did not have the parent's signature for the Emergency Authorization.

746.1107(2) - Additional Minimum Qualifications for Caregivers - HS Diploma, Equivalent, Certification of Completion

2 out of 7 personnel files did not have a high school diploma on file for review.-

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

During the inspection, there were 2 rocking toys and a slide on the playground that did not have loose fill around it. The caregiver removed the toys from the playground during the inspection.

746.3407 - Maintenance of Building, Grounds and Equipment

During current inspection, there was a broken toys with sharp edges observed in the playground the children were in. The caregiver removed the toy during the inspection.

746.2207(a) - Screen Time Activities - Not Used for Under Two

During the inspection, an IPAD was observed being utilized with cartoons showing, while children under 2 years old were in care. The caregiver turned off the IPAD during the inspection.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

2 out of 7 personnel files were missing affidavit form 2985 and 5 out of 7 personnel files were missing affidavit form 2912.

746.3607(2) - Critical Injury - Give First Aid or CPR

Based on the evidence from the DFPS investigation, a child in care received an injury that required medical attention, and the caregiver did not provide first aid.
