Evidence gathered during this investigation shows a child in care was left unsupervised on the playground.

746.305(a)(6) - Report Situation Placing Children at Risk

Evidence gathered during this investigation shows the operation failed to report two incidents where children were placed at risk to Licensing.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Evidence gathered during this investigation shows a caregiver was on her phone while in direct care of children on the playground.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Evidence gathered during this investigation shows a caregiver pulled a child up by the hand/wrist and continued to walk without the child's feet being on the ground.

746.5625(d)(2) - Electronic Child Safety Alarm - Driver or designated employee conduct walk-through and visual check of vehicle

The electronic child safety alarm is installed at the side of the van and not in the back like it is required.

746.5603(c) - Types of Vehicles Used to Transport Children - Be Maintained Safely

The van used for transportation does not have a registration sticker on the front windshield.

746.2206(1)(A) - Written Activity Plan - Outdoor Play - Infant Birth to 12 Months

Infants are not being given two outdoor play opportunities daily when weather permits.

746.3701(1) - Safety - Electrical Outlets Covered

An outlet was accessible to younger children in care in a classroom. This was immediately corrected when an outlet cover was placed over the exposed outlet.

746.3803(a)(1) - Medication Authorization - Signed and Dated

Three children medication records did not have parental signature authorizing medication administration.

746.3701 - Safety - Areas Free From Hazards

Two shelves in the infant classroom presented a tipping hazard should the infants in care pull-up on the shelving. The infants in care had access to the shelves as the shelves had toys on them for the infants in care.

746.3407(8) - Garbage Inaccessible to Children; Maintained in Sanitary Manner

Trash cans utilized by younger children in care did not have lids making trash accessible to children in care.

746.3701(9) - Safety - Bodies of Water Inaccessible to Children

A large plastic tub of standing gray water was observed under a sink in the bathroom utilized by children in care. The washer back flowed into the children's sink in the bathroom, causing concern that the children in care had access to contaminated gray water in conjunction with standing water. This was immediately corrected during the inspection when the tub of water was emptied and the children in care no longer had access to the bathroom temporarily.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

During the inspection numerous hazards were observed. Ceiling tiles located in the classrooms, dining area, hallway, and bathroom had water damage and/or the ceiling tile was not intact. Numerous tiles were missing from the wall in the bathroom children in care utilize, exposing them to debris and unknown substances on the wall.

746.5311(a) - Smoke-Detection System

Two classrooms utilized by children in care did not have working smoke detectors.
