746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Evidence gathered during this investigation shows a caregiver did not use good judgment when dealing with children. Evidence shows a caregiver pulled a child's hair and also told another child to bite a child back after a biting incident.

746.2424 - Infants sleep in Designated Crib, Cot, Bed, or Mat

2 infants were observed to be sleeping on pillows that were on the floor in the infant room. This was corrected at inspection when the infants were removed from the pilows and placed in their cribs.

746.3417(3) - Children Handwashing After Toileting or Diapering

A caregvier was observed changing diapers on the playground without washing the child hands or the caregviers hands. Only wipes were being used for handwashing.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

One child record surveyed out of four did not have a current immunzation record.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File

A 7 week old infant in care was swaddled while sleeping in the crib. This was immediately corrected during the inspection by removing the swaddling device.
