During the inspection the files of two out of eight child files did not have immunization records.

Two infants who do not eat table food did not have infant feeding instructions on file during the inspection.

One caregiver's record did not include orientation training in handling, storage and disposing of hazardous materials.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver was present at the operation who had a pending background check. The caregiver was sent home at the inspection.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

During the inspection disinfectant spray was observed sitting on a low bookcase which was accessible and within reach of children. This was corrected during the inspection when the product was put on a high shelf, out of the reach of children.

746.3701(1) - Safety - Electrical Outlets Covered

During the inspection five electrical outlets were uncovered in the pre-k classroom and one electrical outlet was uncovered in the two-year-old classroom. This was corrected during the inspection when staff covered the electrical outlets with outlet covers.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

This standard was evaluated during the investigation and was found to be deficient. The use zones surrounding the active play structure do not have loose fill or unitary equipment installed.

746.4701(c) - Requirements for Swings - Not Attached to a Composite Structure

This standard was evaluated during the investigation and was found to be deficient. The play structure on the operation's playground has swings attached to a composite structure.
