746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

During the investigation, video footage was reviewed which showed that two fourteen-month-old infants were placed in a restrictive device for approximately thirty minutes. The infants were not eating during this time.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

During the investigation, video footage was reviewed which showed that infants were placed with a group of children who included a six-year-old child.

This standard was evaluated during the investigation and was found to be deficient. The operation did not notify the parent of that the child was scratched and had a bruise.

This standard was evaluated and was found to be deficient. Caregivers did not properly supervise children who were roughhousing while in care.

A child left the facility unsupervised and ran down a nearby street to their parent who was driving away in a vehicle, while the childcare staff was unaware. Staff were made aware of this when the parent brought the child back into the building.

This standard was evaluated during the investigation and was found to be deficient. The operation's management did not report an incident in which a child left the facility unsupervised.

This standard was evaluated during the investigation and was found to be compliant. The operation did not provide an incident report to the parent regarding the situation in which a child left the facility unsupervised.

This standard was evaluated during the investigation and was found to be deficient. Caregivers did not provide adequate supervision ensure an infant's safety when an school-aged child picked the infant up by the arms and accidently dropped the infant from the infant's standing position, causing the infant to hit their head on the ground.
