A caregiver was caring for 5 infants in the infant room. The ages of the children were a 2 month old, two infants 5 months old, a 9 month old, and a 14 month old. The specified age of the children in the group is 0-11 months. According to the ratio chart the maximum number of children one caregiver may supervise is 4.

A caregiver was caring for 11 children of varying ages including infants in the outdoor playground. There was more than an 18 month age difference between the oldest and youngest child in the group. The youngest child in the group was 2 months old and the oldest child was 4 years old.

The diaper changing mat located in the pre-k room was covered with a type of material that was absorbent and hard to sanitize. Note: This was CORRECTED during the inspection as the cover was removed from the diaper changing mat.

745.8411(b)(4) - Responsibilities - Not delay or prevent inspection or investigation

The operation delayed CCR from conducting an inspection when staff took approximately 6 minutes to open the door to the center after knocking, ringing doorbell, and calling the operation's phone number.

746.2205(a)(1) - Written Activity Plan - Group Name and Dates

The activity plan posted in the preschool room was not current. The dates on the activity plan was December2024. Note: This was CORRECTED during the inspection as an updated activity plan was posted.

746.3407 - Maintenance of Building, Grounds and Equipment

Some areas of the wooden fence in the outdoor playground had peeling paint. This posed a hazard as the areas where the paint was peeling was within children's reach.

746.4309 - Fenced Outdoor Activity Space -Gates Locked

A caregiver caring for a group of children in the outdoor playground did not have the key accessible to the lock on the emergency exit gate. A second caregiver that also went to the outdoor playgorund with chlidren at a different time during the insepction did not take the key to the lock on the gate.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

The director did not have current training on Transportation Safety. The training certificate available for review had a date of completion of January 04, 2022.
