An 8 month old infant was observed asleep on a restrictive device (Floor Bouncer) for an undetermined amount of time. Note: The infant was moved to their assigned play yard during inspection.

746.603(a) - Children's Records Maintained

1 of 8 children's records reviewed was missing the statement of the child's health from a health-care professional. A second child's record was missing the child's immunization record.

746.1111 - Education Outside of U.S.

A caregiver's education documentation received outside of the United States was not translated into English.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

A total of 8 children ages 15mo-3yo were combined in room #2. Age difference between youngest and oldest child in the room exceeds 18 months.

746.1701 - Child/Caregiver Ratio - 12 or Fewer Children

One caregiver was supervising a group of 9 children ages 15mo, 2yo, 2yo, 2yo, 2yo, 3yo, 3yo, 3yo and 3yo. Classroom was over ratio by 3 children. Note: This was corrected when an additional caregiver arrived 26 minutes later.

746.901(6) - Required Personnel Records -Training Hours

Two out of six staff training records reviewed and missing certificates for the following training topics: hazardous materials, food allergies, emergency preparedness and communicable diseases.

Based on the information obtained during the course of the investigation, children under 13 years told watched PG13 rated movies / videos.
