A person with a pending background check was observed at the operation.The person was asked to leave by the operation.

747.3203 - Sanitation of Building, Grounds and Equipment

I observed a room that will be utilized by children to have storage bins stacked high, they present a safety risk of falling on the children. In the backyard, I observed planks of wood, a wheel barrel and a ladder that presented a risk to children. The grass was also observed not have been mowed.

747.1503(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

As I arrived for the inspection a 4 year old child was observed to have opened the door, while the caregiver was in the bathroom. I waited at the door for approximately 3-4 minutes before the caregiver came to the door. The door unlcked presented a safety risk to the children as the children could have walked out the door without anyone noticing.

747.3703(a) - Animals at Center - Documentation of Vaccinations

There was a cat in the home with a vaccination record from September 2023. The animal had not been to the veterinarian 2024.

747.1309(a) - Primary Caregiver Annual Training -30 Hours Required

The caregiver was unable to provide documentation of any annual trainings completed in 2023.

747.3203 - Sanitation of Building, Grounds and Equipment

In the restooom, the based of the toliet was observed to be full of dried utine.

747.2321(b) - Written Feeding Instructions - Review and Update Every 30 Days

There was 1 child in care that requred an updated infant feeding instruction form. The form on file was observed to have been predated and signed through January 2025.

747.3501 - Safety - Areas Free From Hazards

In the backyard I obseved a pile of toys near the exit, which presented a safety hazards when children attempted to complete a fire drill and walk safetly to the end of the fence. In the backyear area, a large gasoline tank was observed accessible to children in care. The fence in the backyard had broken fence planks and exposed nails.

747.2101 - Planned Activities - Designed to meet individual needs and developmental levels of children in the group

There was no activity schedule to review. The children were observed running around, asking for the television and were observed fighting with each other. The activity plan was requested and the provider did not have one for review.

747.3501(1) - Safety - Electrical Outlets Covered

There were 4 outlets that were observed to not have an outlet cover. The caregiver placed covers on the outlets during the inspection.

747.603(a)(3) - Children's Records - Health Statement

There was 1 children's file that did not have a health statement on file.

747.501(9) - Written Operational Policies - Parent Rights

The operational policy was reviewed and the parent rights were observed to have not been included in the policy.

747.3211(7) - Caregiver Handwashing After Handling or Cleaning Body Fluids

The caregiver was observed to have cleaned a child's nose and did not wash their hands afterwards;

747.5005(1) - Fire Drill Monthly - Exit in 3 Minutes

The operation had not completed a fire drill since August of 2024. A fire drill was completed during the inspection to bring the deficiency into compliance.

747.2105(a) - Screen Time Activities - Not Used for Under Two

I observed a television on in the living room, cartoons were observed to have been playing while there were 3 children under the age of 2 years old present.
